Elevating Food is Medicine in Central MA

Community Servings, a co-convener of FIMMA, was awarded a $250,000 Synergy Initiative Planning Grant from the Health Foundation of Central Massachusetts (HFCM) to strengthen and support Central Massachusetts’ regional Food Is Medicine providers and expand access to Medically Tailored Nutrition Services across the entire region.

Expanding MTM Nutrition Services into Healthcare through a three-part strategy:

  1. Identify opportunities for expanding Food Is Medicine Services in Central Massachusetts in collaboration with healthcare partners and community based organizations and establishing sustainable reimbursement pathways
  2. Strengthening and expanding the FIMMA Coalition’s Central Massachusetts representation to advocate for new strategies/opportunities from Part 1 and the Food is Medicine policy changes at state and federal levels necessary to sustain them.
  3. Evaluating the new strategies/opportunities from Part 1 to enhance MTM programming and provide evidence for policy expansion.

Medically Tailored Meal Intervention:

MTMs are personalized for individuals with severe, complex, and chronic illnesses, designed to improve health outcomes, lower costs, and increase patient satisfaction. They are an ideal form of nutrition therapy for patients experiencing multiple chronic health conditions, who can be some of the highest need, highest cost patients to treat, generating up to 50% of overall healthcare costs.

Medically Tailored Food Boxes:

Medically Tailored Food Boxes include a selection of minimally processed grocery items selected by a Registered Dietitian or other qualified nutrition professional as part of a treatment plan for an individual with a defined medical diagnosis. The recipient of a medically tailored meal box is typically capable of shopping for and picking up the food and preparing it at home, and is referred by a healthcare provider or plan

Pilot

The Medically Tailored Food Box (MTFB) proposed pilot project will recruit 50-55 Central Massachusetts residents with chronic or critical illnesses using evidence-based strategies including transitions from Medically Tailored Meals (MTM), direct referrals from healthcare partners, and individuals ineligible for MTMs due to lack of illness severity. Beginning in January 2025, participants will receive 12 biweekly home-delivered food boxes, including ingredients for 20 meals and nutrition education led by Registered Dietitian Nutritionists (RDNs).

The intervention will empower participants to prepare meals that meet their health needs, improve food security, and support long-term well-being. This pilot fills a gap in the region’s Food Is Medicine (FIM) services by providing a less intensive, home-delivered solution. The pilot leverages the Central Massachusetts region’s agricultural resources, integrates local food sourcing, and builds on Community Servings’ extensive research demonstrating the effectiveness of MTMs in improving health outcomes and reducing healthcare costs.

Synergy Evaluators:

Kurt Hager is an Instructor in the Division of Epidemiology, Department of Population and Quantitative Health Sciences. Dr. Hager’s interests lay at the intersection of structural determinants of health, food insecurity, and government nutrition and health insurance programs. His current research focuses on the effectiveness of nutritional interventions and policies on chronic disease in the U.S, including evaluations of produce prescriptions and medically tailored meals integrated into clinical care.

Matthew Alcusky is an Associate Professor in the Division of Epidemiology, Department of Population and Quantitative Health Sciences. As a pharmacoepidemiologist and health services researcher, he is primarily focused on generating evidence from large observational data sources to inform clinical practice and guide health policy.

Medically Tailored Meals

Medically tailored meals (MTM) are meals developed to address the dietary needs of an individual’s medical condition by a Registered Dietitian Nutritionist. Individuals are referred by a healthcare provider or plan.

Medically Tailored Food Packages

Medically tailored food packages (MTFP) include a selection of minimally prepared grocery items selected by a Registered Dietitian Nutritionist or other qualified nutrition professional as part of a treatment plan for an individual with a defined medical diagnosis. The recipient of a medically tailored food package is typically capable of shopping for and picking up the food and preparing it at home, and is referred by a healthcare provider or plan.

Produce Prescriptions/Produce Prescriptions+

Produce prescriptions/produce prescriptions+ (PRx/PRx+) provide funds for free or discounted nutritious foods. Individuals must receive referrals from healthcare providers or plans after being identified as having or being at risk for diet-related diseases.

 

Community-Level Healthy Food Programs

Community-level healthy food programs (CLHFP) provide nutritious foods for a population that currently has or is at increased risk for chronic disease associated with food insecurity. These programs are done in partnership or consultation with a healthcare payer, provider or nutrition professional, for screening or referrals, and/or in program design, management, or evaluation.
